Provent Therapy Prescribing Information

Provent Therapy is a one size fits all, single-use disposable device indicated for the treatment of obstructive sleep apnea. It is available as a 10-night starter kit and a 30-night monthly supply.

Prescribing Provent Sleep Apnea Therapy

You can prescribe a 10-night starter kit and/or a 30-night supply for your patient. Once the supplier receives your prescription, the patient will be contacted to confirm shipment and collect payment.

Provent may or may not be covered by the patient's insurance. However, Edgepark Medical, a national mail order provider, may submit claims on the patient's behalf.

To prescribe Provent Therapy:

1. Complete prescription/doctor's order (choose one option)

2. Fax order form to your preferred Provent Therapy supplier or Edgepark Medical
